adding Deaccession record leads to white screen

Description
Create an acession record or navigate to an existing one
Click "Deaccession" in the button block, and add data to the deaccession record. Click save.
Resulting error
Screen goes white - no error message. Deaccession record appears to be created, but screen does not reroute user.
Expected result
Deaccession showscreen or related Accession showscreen.
