Imports cause creator name to appear next to dates

It seems that when data is imported into AtoM, something in our import script is causing the name of the creator to display next to the dates (see attached screenshot). This is different than the behavior when a record is created in AtoM. While RAD does use the events module to include creator names and dates together, other templates such as ISAD and DACS have distinct fields for this data, and the creator name should not be appended to the dates field - nor should the behavior be different for born-AtoM records versus imported ones.
I managed to reproduce this behavior with both a CSV and an EAD XML import. Sample import files attached.
Assigning to Mike Gale because he has become the de facto import king - but I would love to see this addressed in time for our 2.1 release......

I think this is related to #4845 and duplicated creation dates - at least in terms of the XML import. It's clearer if you switch to the RAD template and look in the edit view - dates are duplicated, with the creator name linked to the second date.
With the CSV import, I didn't get duplicated dates, but the name of the creator is linked to the dates.
Looking at https://github.com/artefactual/atom/blob/qa/2.1.x/plugins/sfIsadPlugin/modules/sfIsadPlugin/templates/indexSuccess.php (starting around line 100), my guess is that when the ISAD edit form was changed to separate out dates of creation from name of creator, the display was not changed accordingly. So the data isn't being appended on import; the two separate fields are being displayed together.
