After extensive testing, it turned out it was the Apache FOP tool itself causing the issue. I tried rendering the PDF with both Apache FOP 1.0 and 1.1. The latter crashes on the .fo file, and I've filed an issue ticket with Apache's bug tracker.

For now the solution is to just stick to FOP 1.0, and these errors should be resolved.
