Include ability to select if slugs are based on title, identifier, or reference code for archival descriptions

This issue will further refine the feature added in #8781. In that issue, we added a setting allowing users to determine if the slug of a new archival description is generated from either the full reference code, or the title.
This new feature will further refine those options.
Previously, the reference code option did not allow users to specify whether or not the country and repo code were part of the slug or not, and just using the local identifier was not an option. Now, with these enhancements, there will be 4 options available in the admin setting:
- Reference code (without country and repository code)
- Reference code (with country and repository code)
As before, if the slug exists, AtoM should automatically append a number for the next available slug. Users can always use the "Rename" module to further refine the slug if needed.
